June 17, 1999 -- In a review article published in this week's issue of The New
England Journal of Medicine, Boston University School of Medicine (BUSM)
researchers discussed significant and potentially fatal side-effects of
nonsteroidal anti-inflammatory drug (NSAID) use. NSAIDs include medications such
as aspirin, ibuprofen (Advil, Motrin), naproxen sodium (Aleve, Naprosyn,
Naprox), diclofenac (Voltaren), indomethacin (Indocin), and many others. M.
Michael Wolfe, MD, professor of medicine at BUSM and chief of the section of
gastroenterology in the department of medicine at Boston Medical Center led the
study. "This is, in many ways, a silent epidemic because gastrointestinal
complications are not preceded by any warning signs in a majority of
individuals," Wolfe said."

"Approximately 1½ hours after the reported ingestion of from 7 to 10 ibuprofen
tablets (400 mg), a 19-month old child weighing 12 kg was seen in the hospital
emergency room, apneic and cyanotic, responding only to painful stimuli. This
type of stimulus, however, was sufficient to induce respiration. Oxygen and
parenteral fluids were given; a greenish-yellow fluid was aspirated from the
stomach with no evidence to indicate the presence of ibuprofen. Two hours after
ingestion the child's condition seemed stable; she still responded only to
painful stimuli and continued to have periods of apnea lasting from 5 to 10
seconds. She was admitted to intensive care and sodium bicarbonate was
administered as well as infusions of dextrose and normal saline. By four hours
post-ingestion she could be aroused easily, sit by herself and respond to spoken
commands. Blood level of ibuprofen was 102.9 mcg/ml, approximately 8½ hours
after accidental ingestion. At 12 hours she appeared to be completely
recovered."

be avoided by patients with a history of asthma attacks, hives or other allergic
reactions to aspirin or other NSAIDs. Rare but severe allergic reactions have

peptic ulcer disease or poor kidney function, since this medication can
aggravate both conditions. Ibuprofen is generally used with caution in patients

toxic blood lithium levels. Children's Advil and Children's Motrin are approved
for use in children, 6 months and older, to treat fever, pain, and symptoms of

to elective surgery because of a mild interference with clotting that is
characteristic of this group of medicines. Ibuprofen is best discontinued at
least two days in advance of the procedure."
