morphine reflects care providers' attention to pain relief.' A third of deaths
in Oregon occur in nursing homes, and studies have shown that 40 to 60 percent
of nursing home patients do not receive adequate pain medication, Tolle said.
"We're pleased to see more attention being given to comfort near the time of
death," she said. Oregon has made it easier to prescribe narcotics by not
requiring cumbersome paperwork, she said. Super encouraged nurses to advocate
pain management for their patients. "We think about pain being in the
physicians' realm, but nurses can have a significant impact," she said."

Morphine
Sulfate: A Healthy Me: "Morphine is habit forming and should only be used
under close supervision if you have an alcohol or drug addiction. Before taking
this medication, tell your doctor if you have kidney disease, liver disease,
asthma, urinary retention, an enlarged prostate hypothyroidism, seizures or
epilepsy, gallbladder disease, a head injury, or Addison's disease. You may not
be able to take morphine, or you may require a lower dose or special monitoring
during treatment if you have any of the conditions listed above. Morphine may
cause addiction and withdrawal symptoms as well as other harmful effects in an
unborn baby. Do not take morphine without first talking to your doctor if you
are pregnant. Morphine may also cause addiction and withdrawal symptoms in a
nursing infant. Do not take morphine without first talking to your doctor if you
are breast-feeding a baby. If you are younger than 18 years of age or older than
60 years of age, you may be more likely to experience side effects from morphine
therapy. Use extra caution.

Structure
of Morphine Sulfate: "Morphine is obtained from opium,
the juice secreted by the seeds of the poppy. It works on several types of
receptors, widely found in nervous tissue. "Opioids" is a term used for all
drugs that act on these receptors. These other opioids may be natural occurring
substances, such as morphine, or made in the laboratory such as pethindine."
Morphine works on receptors in the cell membranes. These are protein-lipid
molecules that alter their shape when stimulated to effect molecular changes
within cells. There are thought to be three types of receptors that respond to
opioids, mu, kappa and lambda. Morphine acts mainly on mu receptors to cause a
wide variety of effects. The most important of these are relief of pain and
respiratory depression."

administration is allowed for medical indications. However, at term there are
significant risks for the neonate (respiratory depression, cardiac problems):
avoid morphine! Nursing mothers: Only small amounts can be traced in breast
milk. Can be given, may suppress lactation! Children: Single dose: 0.1 to 0.2
mg/kg (i.m./s.c. every 4 hours), as infusion 10-30 µg/kg/hour. Long half-lives
in neonates! (Fentanyl may be better tolerated.) Elderly people: Elderly people
have a stronger reaction to morphine: reduce dose! Renal failure: A dose
reduction is recommended (active metabolites may be important). Liver
insufficiency: Generally no problem. A dose reduction is nevertheless
recommended if disease is advanced"
