Amphotericin B, (Fungizone), HIVdent:"Treatment of deep-seated mycotic infection, including cryptococcosis, histoplasmosis, blastomycosis, coccidiomycosis, candida, and aspergillosis (invasive disease). Dose: Highly individualized. Test dose with 1-5mg infusions. If test dose is tolerated, dose at 0.4-0.6mg/kg/day IV slowly over 24 hours. Average daily dose is 50-70mg/day. Duration of therapy depends on the infection. Prehydrate, may require potassium and magnesium. Do not use IV filters with any formulation of amphotericin."

For the first two weeks of treatment, the drug amphotericin B (Fungizone®) is given every day through an IV line, along with a second drug taken by mouth: flucytosine (Ancobon®). Amphotericin B can cause side effects, some of them serious. Side effects include nausea, fever, chills, muscle pain, low potassium levels, damage to the bone marrow and its ability to produce red blood cells and white blood cells, and kidney damage. Tip: Take a regular dose of acetaminophen (e.g., Tylenol®), ibuprofen (e.g., Advil®), naproxen (e.g., Aleve®), or diphenhydramine (e.g., Benadryl®) approximately half an hour before receiving amphotericin B – this can help prevent/reduce some side effects during and after receiving the infusion."

Amphotericin B binds to specific chemical structures--sterols--of the fungal cellular membrane, increasing cellular permeability and promoting loss of potassium and other substances. Liposomal encapsulation or incorporation in a lipid complex can significantly affect the functional properties of the drug compared with those of the unencapsulated or non-lipid-associated drug. The liposomal amphotericin B product causes less nephrotoxicity. Amphotericin B is used either IV or topically. It is highly bound to serum protein (90%) Peak plasma levels: 0.5-2 mcg/mL. t1/2, initial: 24 hr; second phase: 15 days. Slowly excreted by kidneys.The kinetics of the drug differ in adults and children and are also dependent on the product used."
