Chronic heart failure (CHF) is the syndrome of breathlessness, fatigue and fluid retention resulting in an impaired ability of the heart to pump properly. Heart failure is over-diagnosed (especially in the elderly) and is best confirmed using cardiac ultrasound [1]. Older people may show an exaggerated drop in blood pressure after the first dose of an ACE inhibitor. Longer-acting ACE inhibitors are given when the patient's tolerance to Captropril is established. This is another medication which can lead to hypotension.
